To Enroll

• If you are a Levels member and decide to participate in this research, you can join through the Levels app or website.
• In the Levels App or the Levels website, you will be asked basic questions to determine your eligibility to join the study.
• If you are eligible for the study and would like to join the study, you are required to complete the electronic consent form by reviewing the form and providing an electronic signature.
• You must also purchase the study CGMs through the Levels website or mobile app.

To Participate

Upon receipt of the study device, you must review all instructions. You can begin active participation by following these steps (provided on the getting started website, which will be linked via URL and insert card within the study kit upon delivery):

1. Download the Levels app and log in to the profile created during enrollment.
2. Download the CGM manufacturer app and create an account.
3. Place the CGM sensor on the back of the arm in accordance with the instructions.
4. Apply the fabric cover over the CGM sensor.
5. Begin a new CGM sensor session in the CGM manufacturer app.
6. Connect the CGM manufacturer app to the Levels app.
7. Verify connection and glucose data production.

Continuous collection of glucose data will commence once the above steps are completed.  Depending on the CGM System, you will be prompted to replace the CGM sensor after a certain number of days.  If a sensor becomes dislodged, you can replace the sensor prior to its scheduled end of life and indicate you did so on the Levels app or contact [email protected]. If this happens, Levels will initiate the shipment of a replacement study kit.
